OCHIN’s Health Center Controlled Network (HCCN)
No-Cost Support that Enhances Clinical Quality, Patient-Centered Care, and Provider/Staff Well-Being
As the largest of its kind in the nation, OCHIN’s HRSA-funded Health Center Controlled Network (HCCN) helps Participating Health Centers (PHCs) advance health equity by optimizing their use of health information technology (HIT) to improve efficiency, boost clinical quality, enhance interoperability, support provider/staff well-being, and foster patient-centered care.
Our unique “learning laboratory” approach allows health centers to learn from others with similar challenges. It includes dedicated consulting and focused cohorts—encouraging collaborative thinking and innovative problem-solving to strengthen the circle of community care.
OCHIN’s HCCN has received a $9M award from the Health Resources and Services Administration (HRSA) to fund our 2022-2025 program cycle. This federal funding is the fourth HRSA award that OCHIN’s HCCN has received, and the largest amount to date. The addition of more than two dozen new health centers to the OCHIN HCCN represents an 18% increase in overall health center participation with the new award.
How It Works
Health Center Controlled Networks (HCCNs) are collaborative groups of health centers addressing operational and clinical challenges related to health information technology (HIT).
They undertake management, financial, administrative, and clinical initiatives that improve care access, care quality, patient safety, and cost efficiency. They also create economies of scale that facilitate specialized training and technical assistance (T/TA), data analytics, and group purchasing benefits.
Many, like OCHIN’s HCCN, are federally funded by the Health Resources and Services Administration (HRSA). OCHIN’s HCCN received its first federal grant award in 2012.
OCHIN’s HCCN guides health centers to drive efficiencies and improve health outcomes through a variety of no-cost resources and dedicated consulting services, including:
On-demand trainings, live webinars, COVID-19 vaccine support programs, and revenue cycle management tools;
Ongoing access to OCHIN’s recognized in-house subject matter experts;
Specialized insights on UDS+ reporting, value-based payment, regulatory compliance developments, change management strategies, and more;
Comprehensive and customizable security risk assessments, optimization support for multiple electronic health records (EHRs), and data analysis.
Enhanced Efficiency, Continuous Quality Improvement
In July of 2022, OCHIN received $9 million in funding from the Health Resources and Services Administration (HRSA) to support federally qualified health centers (FQHCs) and FQHC look-alikes across the country.
This three-year award currently makes OCHIN’s HCCN the nation’s largest—reaching primary care sites for millions of patients in rural and metropolitan communities from coast to coast.
Our PHCs have the opportunity to improve patient outcomes through peer-to-peer collaboration and health information exchange (HIE) initiatives. These no-cost resources and activities help dedicated care teams use electronic health records (EHRs) more efficiently, improve data analytics, and achieve a higher standard of patient care to advance health equity.
During the 2022-2025 program cycle, Oregon Primary Care Association (OPCA) will partner with OCHIN as a sub-awardee. OPCA will augment OCHIN’s offerings by providing a range of specialized services—including individualized PHC assessment, work plan consultation and development, consulting support, data aggregation, and more.
